摘要:
目的:观察推拿特定穴治疗便秘疗效及对患儿肠动力的影响。方法:选取便秘患儿80例,以随机数字表法分成清大肠组(n=30),下推七节骨组(n=30)以及联合组(n=20),给予清大肠组患儿清大肠推拿,下推七节骨组给予患儿下推七节骨治疗,联合组则予以清大肠联合下推七节骨进行推拿治疗。比较三组患儿临床症状改善情况、临床疗效,并比较治疗前后三组患儿胃肠电图仪检查指标以及胃肠激素[P物质、胃动素(MTL)、一氧化氮(NO)]水平。结果:治疗后,联合组便秘症状评分低于清大肠组与下推七节骨组(P<0.05); 粪便性状分级均增加,且联合组>下推七节骨组>清大肠组(P<0.05)。三组临床疗效比较,联合组(90.00%)>下推七节骨组(83.33%)>清大肠组(60.00%)。治疗后三组患儿降结肠与乙状结肠的波幅及频率均增加,且联合组最高,下推七节骨组次之,清大肠组最低(P<0.05)。另外治疗后三组患儿的MTL与P物质水平均上升,且联合组水平高于清大肠组与下推七节骨组(P<0.05); 三组患儿的NO水平均下降,且联合组水平低于清大肠组与下推七节骨组(P<0.05)。结论:清大肠推拿联合下推七节骨治疗小儿便秘疗效确切,可显著改善患儿症状,同时有利于改善肠动力及胃肠激素水平。
Abstract:
Objective:To observe the effect of massaging specific acupoints in the treatment of children with constipation,and the impact on intestinal motility.Methods:A total of 80 children with constipation were selected.They were divided into Qingdachang group(n=30),pushing down Seven-bone line group(n=30)and combination group(n=20)by random number table method.The three groups were given Qingdachang massage,massage through pushing down Seven-bone line and combined massage therapy of the two,respectively.The three groups were compared in terms of the improvement of clinical symptoms,clinical efficacy,the levels of electrogasfrogram examination indexes and gastrointestinal hormones [substance P,motilin(MTL)and nitric oxide(NO)] before and after treatment.Results:After treatment,constipation symptom scores of the combination group were lower than those of the other two groups(P<0.05).The grade of fecal character increased,and decreased in order from the combination group,the pushing down Seven-bone line group to the Qingdachang group(P<0.05).The clinical response rate decreased in order from the combination group(90.00%),the pushing down Seven-bone line group(83.33%)to the Qingdachang group(60.00%).After treatment,the amplitude and frequency of descending colon and sigmoid colon were increased in the three groups,which were the highest in the combination group,followed by the pushing down Seven-bone line group and then the Qingdachang group(P<0.05).After treatment,the levels of MTL and substance P in the three groups were increased,and the levels in combination group were higher than those in the other two groups(P<0.05).NO levels in the three groups was decreased,and the level in combination group was lower than those in the other two groups(P<0.05).Conclusion:Qingdachang massage combined with pushing down Seven-bone line is effective in the treatment of children with constipation,which can significantly improve clinical symptoms,intestinal motility and gastrointestinal hormones.
