摘要:
小胶质细胞在缺血性脑卒中(IS)的病理过程中既参与炎性反应,又在神经修复中发挥关键作用,是治疗IS的潜在靶点。铁死亡作为由铁过载引发的程序性细胞死亡,已被证实在小胶质细胞中通过芬顿反应催化脂质过氧化及炎症因子释放,加剧神经损伤。针刺作为多靶点干预策略,通过重塑小胶质细胞极化平衡、恢复铁稳态、抑制脂质过氧化及调控神经炎症微环境,有效阻断铁死亡进程,保护神经功能。具体机制包括调控表型转化相关信号通路、调节铁死亡相关蛋白和基因表达、激活内源性抗氧化系统以及干预炎症介质释放。现综述小胶质细胞铁死亡在IS中的作用,分析针刺干预的最新研究进展,强调针刺通过多维度协同效应在减轻铁死亡和改善预后中的潜力。
Abstract:
Ischemic stroke (IS),a major contributor to global disability and mortality,involves neuronal death and neuroinflammation.Ferroptosis,an iron-dependent cell death driven by oxidative stress and lipid peroxidation,playing a central role.Microglia exacerbate neuronal injury through iron overload and inflammatory cascades,creating a self-perpetuating cycle of damage.Acupuncture demonstrates therapeutic efficacy by targeting microglial ferroptosis through multi-modal mechanisms.It modulates microglial polarization toward anti-inflammatory phenotypes,enhances iron homeostasis through regulating iron transporters and redox balance,and suppresses oxidative stress and inflammatory signaling pathways.Acupuncture alleviates neuronal injury through multifaceted mechanisms,regulating microglial polarization and iron balance,curbing lipid peroxidation via reinforced antioxidant systems,attenuating neuroinflammation by suppressing key inflammatory signaling,and enhancing mitochondrial integrity through optimized energy metabolism and dynamics,synergistically mitigating ferroptosis-mediated damage.This article reviews the role of microglial ferroptosis in IS,analyzes the latest research progress of acupuncture intervention,and emphasizes the potential of acupuncture in reducing ferroptosis and improving prognosis through multi-dimensional synergistic effects.
