摘要:
慢性萎缩性胃炎(CAG)是临床上常见的一种慢性胃部疾病，主要表现为反复发作且无规律上腹部疼痛不适或者胀满疼痛等。对于CAG，西医缺乏明显有效的治疗手段，而中医从整体观念论治，对于CAG的治疗具有明显的优势。通过阐述“土壅木郁”的理论内涵，从而探讨慢性萎缩性胃炎合并焦虑状态的发病机制，认为“脾虚土壅”是CAG合并焦虑状态的发病之本，而“木郁不畅”是CAG合并焦虑状态发病的关键之因。而HP感染、炎症免疫反应异常、脑肠轴功能失调在一定程度上反映了“土壅木郁”的状态，影响了CAG 合并焦虑状态的发生发展。治疗当以健运脾气贯穿治病的整个过程，配合疏肝通络治法，CAG病情迁延不愈，病程较长，久病必瘀，治病后期当以配合祛瘀通络之法，佐以心理疏导，调畅情志。
Abstract:
Chronic atrophic gastritis (CAG) is a common chronic gastric disease in clinical practice,mainly characterized by recurrent and irregular upper abdominal pain,discomfort or distension and pain.Western medicine lacks effective therapy for CAG,while traditional Chinese medicine has obvious advantages based on the holistic concept.By elaborating on the the theoretical connotation of “earth stagnation and wood depression”,this article explores the pathogenesis of CAG combined with anxiety state,and holds that “spleen deficiency and earth congestion” is the root cause of CAG combined with anxiety state,while “wood depression and lack of smoothness” is the key factor.Helicobacter pylori (HP) infection,abnormal inflammatory immune response,and brain-gut axis dysfunction to some extent reflect the state of “earth stagnation and wood depression”,which affects the occurrence and development of CAG combined with anxiety state.The treatment should focus on invigorating and regulating the spleen qi throughout the entire treatment process,combined with the method of soothing the liver and unblocking the meridians.As CAG has a prolonged course and is difficult to cure,long-term illness leads to blood stasis.In the later stage of treatment,it is necessary to combine the method of removing blood stasis and unblocking the meridians,and assist with psychological counseling to regulate emotions.
