摘要:
丁霞老师基于“治中焦如衡”理论指出慢性萎缩性胃炎(CAG)是由中焦失衡，气机升降失调，运化水谷功能失司所致。中焦脾胃具有“升降相因、纳运相依、燥湿相济”的生理特性,而“衡”的状态是功能运行的基础。因此，中焦失衡，精微物质难以布散且糟粕滞留，进而导致气滞、痰浊、瘀血等壅滞体内，在治疗上可分期论治。丁霞老师认为CAG初期以正虚邪生，升降失衡为主，治疗当以益气健脾，升降同调以复衡为基本治法；随着时间迁移，患者病情不断发展，长期脾胃气虚造成中阳虚损，加之体内痰浊、水湿停滞日久，留滞脾胃，郁而化热，渐成寒热错杂之证，故须化湿和中，寒热同调以复衡；在CAG后期,胃络受损,瘀血阻滞,气血失衡,辨治应详查其因，气血同调以复其衡。
Abstract:
Based on the theory of “treating middle Jiao as balance”，Professor DING Xia points out that chronic atrophic gastritis (CAG) is caused by imbalance of the middle Jiao,disordered ascending and descending of qi,and dysfunction in transporting and transforming water and grains.The middle Jiao,referring to the spleen and stomach,is characterized by physiological properties of “ascending and descending interdependence,reception and transportation reliance,and dryness-dampness harmony”.The state of “balance” serves as the foundation for its functional operations.Therefore,imbalance in the middle Jiao leads to impaired distribution of essence and retention of waste,subsequently causing qi stagnation,phlegm turbidity,and blood stasis to accumulate internally.In treatment,a staged approach can be adopted.Professor DING Xia believes that the initial stage of CAG is dominated by deficiency of healthy qi and occurrence of pathogenic factors,along with imbalance of ascending and descending.The primary treatment method should focus on tonifying qi,invigorating the spleen,and restoring balance by regulating both ascending and descending functions.As the disease progresses over time,prolonged spleen and stomach qi deficiency leads to yang deficiency in the middle Jiao.Additionally,accumulated phlegm turbidity and dampness in the body stagnate,transform into heat,and gradually develop into a mixed cold-heat syndrome.Therefore,treatment must focus on resolving dampness,harmonizing the middle,and balancing cold and heat to restore equilibrium.In the late stage of CAG,damaged stomach collaterals,blocked blood stasis,and imbalanced qi and blood require detailed examination of the underlying causes,with concurrent regulation of qi and blood to restore balance.
