摘要:
糖尿病肾病(DN)是糖尿病的高发并发症之一,其病程迁延难愈。肾脏M1/M2型巨噬细胞极化失衡与肾脏免疫紊乱、肾组织炎症与纤维化密切相关;“血不利则为水”是中医阐释血水代谢的经典理论,揭示血瘀与水停的病理关联,而巨噬细胞的极化过程与该理论高度契合。基于此探讨“血不利则为水”、巨噬细胞极化与DN的关系,认为“肾络瘀滞”为其病因之本,“水湿内停”为其病因之标。“肾络瘀滞”核心源于脾肾气阴亏虚,“水湿内停”主要是瘀、湿、浊毒留滞,治法以补肾活血、健脾利湿、化浊解毒、养阴清热为主,以达血水调和,为DN的诊治提供思路。
Abstract:
Diabetic nephropathy (DN) is one of the highly prevalent complications of diabetes mellitus,characterized by a protracted and refractory clinical course.The imbalance of M1/M2 macrophage polarization in the kidney is closely associated with renal immune disorder,renal tissue inflammation and fibrosis.The theory of “unsmooth blood circulation results in water” is a classic traditional Chinese Medicine doctrine illustrating the metabolism of blood and body fluid,revealing the pathological correlation between blood stasis and fluid accumulation.Notably,the process of macrophage polarization is highly consistent with this theory.Based on this,we explore the relationship among “unsmooth blood circulation results in water”,macrophage polarization and DN,proposing that “renal collaterals stasis” serves as the fundamental etiology,while “internal retention of water-dampness” acts as the superficial manifestation.The core mechanism of “renal collaterals stasis” originates from Qi-Yin deficiency of the spleen and kidney,and “internal retention of water-dampness” is mainly caused by the stagnation of blood stasis,dampness and turbid toxin.The therapeutic principles include tonifying the kidney and activating blood circulation,invigorating the spleen and resolving dampness,eliminating turbidity and detoxifying,as well as nourishing Yin and clearing heat,so as to achieve the harmony of blood and fluid,thereby providing novel insights for the diagnosis and treatment of DN.
