摘 要 目的：研究普瑞巴林联合桃红四物汤加减治疗带状疱疹后遗神经痛（PHN）的疗效及机制。方法：将134例PHN患者按密封信封法随机分为两组，均加以常规对症治疗，在此基础上单一组67例加以普瑞巴林治疗，联合组67例加以桃红四物汤加减治疗，治疗4周。观察两组临床疗效、疼痛程度、睡眠质量、Th1/Th2细胞因子与血清炎症因子水平以及生活质量的变化，并评价两组治疗安全性。结果：治疗后，联合组与单一组总好转率分别为92.54%与79.10%，联合组临床疗效显著优于单一组(P＜0.05)。两组VAS评分、PSQI评分明显降低(P＜0.05)，联合组下降幅度大于单一组(P＜0.05)。两组IL2、IL10显著升高(P＜0.05)，IL1β、TNFα、MCP1显著降低(P＜0.05)，联合组变化幅度大于单一组(P＜0.05)。联合组评分的上升幅度大于单一组(P＜0.05)。两组心肝肾功能检查正常，不良反应发生率差异无统计学意义(P＞0.05)。结论：普瑞巴林联合桃红四物汤治疗PHN，可显著提升治疗效果，减轻患者疼痛程度，改善睡眠状况，调节Th1/Th2细胞因子与血清炎症因子水平，提高患者生活质量，疗效较好。
Abstract:
Abstract Objective: To study the efficacy and mechanism of Taohong Siwu decoction combined with pregabalin in the treatment of postherpetic neuralgia (PHN). Methods: A total of 134 patients with PHN were randomly divided into two groups according to the sealed envelope method. All of them were treated with conventional symptomatic treatment. On the basis of this, 67 patients in single group were treated with pregabalin, and 67 patients in combined group were additionally given addition and subtraction of Taohong Siwu decoction. At 4 weeks of treatment, the clinical efficacy, pain level, sleep quality, Th1/Th2 cytokine and serum inflammatory factors levels and quality of life were observed in the two groups, and the treatment safety was evaluated in the two groups. Results: After treatment, the total improvement rates in combined group and single group were 92.54% and 79.10% respectively, and the clinical efficacy in combined group was significantly better than that in single group (P＜0.05). The VAS score and PSQI score in the two groups were significantly decreased (P＜0.05), and the decreases in combined group were greater than those in single group (P＜0.05). The levels of IL2 and IL10 were significantly increased in the two groups (P＜0.05) while the levels of IL1β, TNFα and MCP1 were significantly decreased (P＜0.05), and the changes in combined group were greater than those in single group (P＜0.05). The scores increases in combined group were greater than those in single group (P＜0.05). The two groups had normal heart, liver and kidney function tests, and there was no significant difference in incidence rate of adverse reactions (P＞0.05). Conclusion: Taohong Siwu decoction combined with pregabalin for PHN can significantly improve the therapeutic effects, reduce the pain level, improve the sleep status, regulate the levels of Th1/Th2 cytokine and serum inflammatory factors, and improve the quality of life of patients, with exact efficacy.
