摘要:
谢晶日老师致力于胃黏膜上皮内瘤变(IN)的研究,认为慢性萎缩性胃炎(CAG)伴上皮内瘤变病位在胃,病源却在肝脾,属本虚标实之证,本虚责之脾气虚、胃阴虚; 标实责之气滞、湿热、瘀毒。基本病机为土虚木乘,肝木克伐脾土,脾胃升降失司以致气滞湿热瘀毒搏结于胃。谢老师在结合前贤经验和数十年临证总结后,强调辨证应化繁为简,以两分法进行辨证,将病机概括为“虚”“积”两方面,提出“补土消积”这一治法,虚者补土,以健脾益胃为本; 实者消积,以疏肝和胃为先,以清热利湿、化瘀解毒为要。贯彻“治中焦如衡,非平不安”的治疗原则,消补结合以平调气血阴阳,同时注重未病先防,强调身心同治,临床疗效甚佳。
Abstract:
XIE Jingri has been committed to the research of gastric mucosal intraepithelial neoplasia(IN)for a long time.He believes that chronic atrophic gastritis(CAG)with intraepithelial neoplasia is in the stomach,but the source is in the liver and spleen,which asthenia in origin and asthenia in superficiality.the deficiency is insufficiency of the spleen qi and stomach yin.The superficially is qi stagnation,dampness-heat and blood stasis. The basic pathogenesis is that the soil is deficient and the wood is abundant,the liver depression spleen and the spleen and stomach are abnormal,resulting in blood and damp heat in the stomach.After combining the experience of the predecessors and decades of experience,professor XIE emphasized that syndrome differentiation should make complex problems simple,and syndrome differentiation should be carried out by dichotomous method.He summarized the pathogenesis as “deficiency” and “stagnation”,and proposed the treatment of “tonification and purgation in combination”.People with deficiency syndrome should replenish soil,take strengthening the spleen and benefiting the stomach as the basis,and people with excess syndrome should first soothe the liver and stomach,and the most important thing is to clear heat and remove dampness,remove blood stasis and detoxify.Implement the treatment principle of “the treatment of middle coke disease requires the use of balance method to achieve better curative effect”,apply both attack and tonic to adjust qi and blood yin and yang,pay attention to prevention before disease,and emphasize the treatment of both body and mind.The clinical effect is good.
