摘要:
目的:探讨温针灸联合经皮椎体后凸成形术治疗骨质疏松性椎体压缩性骨折的疗效以及对骨密度(BMD)及血钙水平的影响。方法:筛选86例骨质疏松性椎体压缩性骨折者作为研究对象,按随机数字表法分为对照组和观察组,各43例。两组患者接受经皮椎体后凸成形术; 对照组患者术后给予抗感染等常规治疗措施; 观察组在对照组的基础上加用温针灸治疗,隔日1次,每周5次,连续治疗12周。比较两组疼痛程度、椎体功能评分、骨折愈合、临床疗效以及血钙和BMD水平。结果:治疗后,观察组患者疼痛视觉模拟评分(1.44±0.25)分和Oswestry功能障碍指数评分(19.09±3.15)分均少于对照组[(2.40±0.41)分、(21.33±4.43)分](P<0.01)。观察组患者治疗后矢状位脊柱后凸Cobb 角(5.25±0.78)°明显低于对照组(12.09±1.99)°,矢状位伤椎前缘高度(20.65±3.72)mm显著高于对照组(14.27±2.75)mm(P<0.01)。观察组的总有效率为97.67%,明显高于对照组的81.40%(P<0.05)。治疗后,观察组患者腰椎正位(L2-4)(0.755±0.087)g/cm2和右跟骨(0.669±0.073)g/cm2的BMD显著高于对照组[(0.722±0.083)g/cm2、(0.649±0.071)g/cm2],血钙(1.81±027)mmol/L明显低于对照组(1.94±0.30)mmol/L(P<0.01)。结论:温针灸联合经皮椎体后凸成形术治疗骨质疏松性椎体压缩性骨折的疗效较好,其疗效可能与调节BMD及血钙水平有关。
Abstract:
Objective:To investigate the efficacy of warm acupuncture combined with percutaneous kyphoplasty in treating of osteoporotic vertebral compression fracture and the effect on levels of bone mineral density(BMD)and serum calcium.Methods:86 osteoporotic vertebral compression fracture patients were selected as the subjects of the study and divided into the control group and the observation group according to the random number table(43 cases for each group).Two groups of patients received the percutaneous kyphoplasty.Patients in the control group were given with routine treatment measures such as anti-infection.The observation group was treated with warm acupuncture(once every other day and 5 times/week)on the basis of the control group for 12 weeks.Scores of pain severity and vertebral function,fracture healing,the clinical efficacy,and levels of blood calcium and BMD were compared between the two groups.Results:After treatment,visual analogue scores(1.44±0.25)score and oswestry dysfunction index scores(19.09±3.15)score in the observation group were significantly lower than those in the control group[(2.40±0.41)score and(21.33±4.43)score](P<0.01).Cobb Angle of posterior sagittal kyphosis(5.25±0.78)° in the observation group was significantly lower,while the height of anterior vertebral edge of sagittal injury(20.65±3.72)mm was significantly higher,than those of the control group[(12.09±1.99)° and(14.27±2.75)mm](P<0.01).The total effective rate of the observation group was 97.67%,significantly higher than that of the control group as 81.40%(P<0.05).After treatment,BMD of the lumbar spine(L2-4)( 0.755±0.087)g/cm2 and right calcaneal bone(0.669±0.073)g/cm2 in the observation group[(0.722±0.083)g/cm2 and(0.649±0.071)g/cm2] was significantly higher,while blood calcium(1.81±027)mmol/L was significantly lower,than those in the control group(1.94±0.30)mmol/L(P<0.01).Conclusion:Efficacy of warm acupuncture combined with percutaneous kyphoplasty in treating of osteoporotic vertebral compression fracture is significant,and regulation of levels of BMD and serum calcium may be related with the efficacy.
