摘要:
糖尿病性周围神经病变（DPN）是糖尿病常见且最难治的慢性并发症之一，以下肢感觉异常、肌力减退为核心表现，严重影响患者生活质量，甚至危及生命。西医多采用营养神经、控制血糖等对症治疗，但疗效不佳，且长期用药存在不良反应。中医疗法讲究整体观念，遵循辨证论治，具有针对性强，不良反应少的优势。为提高DPN的临床疗效，中医药在DPN治疗中逐步推广，如中药内服、中药外治（针灸、穴位贴敷、中药沐足）等，在改善DPN患者症状、延缓疾病进展方面展现出独特价值。本研究系统梳理近年文献，从病名源流、病因病机、辨证分型及对应方药、中医外治疗法等方面展开，明确各证型的治则治法、方剂配伍规律及现代药理机制，分析当前研究的不足，提出针对性改进方向，为 DPN中医规范化诊疗提供循证依据与理论支撑。
Abstract:
Diabetic peripheral neuropathy (DPN) was one of the common and most difficult-to-treat chronic complications of diabetes,with core manifestations including abnormal sensation and muscle strength reduction in the lower extremities,which seriously affects the quality of life of patients and even endangers their lives.Western medicine mostly adopts symptomatic treatments such as nerve nutrition and blood sugar control,but the therapeutic effect was not satisfactory,and long-term medication has adverse reactions.Traditional Chinese medicine (TCM) emphasizes the holistic concept and follows the principle of syndrome differentiation and treatment,which has the advantages of strong targeting and few adverse reactions.To improve the clinical efficacy of DPN,TCM has been gradually promoted in the treatment of DPN,such as oral administration of traditional Chinese medicine,external treatment with traditional Chinese medicine (acupuncture,moxibustion,acupoint application,traditional Chinese medicine foot bath),which have shown unique value in improving the symptoms of DPN patients and delaying the progression of the disease.This study systematically reviews the recent literature and conducts research from aspects such as origin of the disease name,etiology and pathogenesis,syndrome differentiation and corresponding prescriptions,and TCM external treatment methods,to clarify the treatment principles and methods of each syndrome type,the rules of prescription composition,and the modern pharmacological mechanisms,analyze the deficiencies of current research,and propose targeted improvement directions,providing evidence-based basis and theoretical support for the standardized diagnosis and treatment of DPN in TCM.
