摘要:
神经病理性疼痛(NPP)是一种难治性疼痛综合征,由躯体感觉神经系统的损伤或疾病引起,以自发性疼痛、痛觉过敏、异常疼痛和感觉异常为特征。星形胶质细胞作为中枢神经系统中最大、数量最多的神经胶质细胞,当其被激活为反应性星形胶质细胞后,对神经病理性疼痛进展起到重大影响,而Ca2+信号是反应性星形胶质细胞的重要参与者。因此,抑制星形胶质细胞活化有可能是缓解神经病理性疼痛的潜在策略。目前临床治疗神经病理性疼痛的方式主要是应用镇痛药物,但往往因现有镇痛药物的疗效有限和不良反应而复杂化。推拿作为一种传统中医治疗手段,因其不良反应少、疗效明显,在疼痛患者中被广泛使用。且推拿对Ca2+信号、星形胶质细胞活化的调控作用也有相关文献记载,但推拿的镇痛机制尚不清楚。因此笔者通过搜索有关文献,分析总结推拿干预Ca2+信号、抑制星形胶质细胞活化,减轻神经病理性疼痛相关信号通路的研究进展。
Abstract:
Neuropathic pain(NPP)is a refractory pain syndrome caused by injury or disease of the somatosensory nervous system,which is characterized by spontaneous pain,hyperalgesia,abnormal pain and paresthesia.As the largest and largest number of glial cells in the central nervous system,astrocytes play an important role in the progression of neuropathic pain when they are activated into reactive astrocytes,and Ca2+ signaling is an important participant in reactive astrocytes.Therefore,inhibition of astrocyte activation may be a potential strategy to alleviate neuropathic pain.At present,the clinical treatment of neuropathic pain is mainly analgesic drugs,but it is often complicated by the limited efficacy and adverse side effects of existing analgesic drugs.As a traditional Chinese medicine treatment,massage is widely used in patients with pain because of its less adverse reactions and obvious curative effect.The regulation of massage on Ca2+ signal and astrocyte activation has also been recorded in the literature,but the analgesic mechanism of massage is still unclear.Therefore,the author analyzed and summarized the research progress of massage intervention in Ca2+ signaling,inhibition of astrocyte activation,and reduction of neuropathic pain-related signaling pathways by searching relevant literature.
