摘要:
目的：探讨基于“络虚通补”的吴门培补二天膏对老年高血压（HTN）患者血压变异性（BPV）及生活质量的影响。方法：按照随机数字表法，将 113 例老年HTN患者分为两组。对照组（n=56）采用常规西药降压治疗；观察组（n=57）在常规治疗基础上加用吴门培补二天膏，治疗周期为 6 周。比较两组患者治疗前后的血压水平、健康调查简表（SF-36）评分、中医证候积分及中医疗效变化，并对比两组患者治疗期间的不良反应。结果：治疗6周后，观察组舒张压标准差（DSD）、24 h 平均舒张压变异系数（DCV）降低（均P＜0.05）。观察组的生理机能、情感职能、精神健康、精力、一般健康状况以及对照组的一般健康状况积分升高（均P＜0.05），且观察组的情感职能积分高于对照组（均P＜0.05）。观察组的眩晕、腰酸、膝软、五心烦热、畏寒肢冷、心悸、失眠、健忘、气短、夜尿频以及对照组眩晕、腰酸、胸闷、畏寒肢冷、口干、失眠、健忘、夜尿频积分降低（均P＜0.05）。治疗12周后，观察组的生理机能、生理职能、情感职能、躯体疼痛、精神健康、精力、一般健康状况以及对照组的精神健康、精力、一般健康状况积分升高（P＜0.05），且观察组情感职能积分高于对照组（P＜0.05）。观察组的眩晕、腰酸、膝软、五心烦热、胸闷、畏寒肢冷、便秘、心悸、失眠、健忘、食少、气短、夜尿频以及对照组眩晕、腰酸、胸闷、畏寒肢冷、口干、心悸、失眠、健忘积分降低（P＜0.05），且观察组的健忘积分低于对照组（P＜0.05）。观察组中医疗效总有效率高于对照组（P＜0.05）。治疗期间，两组患者均未出现严重不良反应。结论：基于“络虚通补”的吴门培补二天膏联合西药治疗，可有效改善老年HTN患者的BPV和生活质量，且安全性较高。
Abstract:
Objective:To explore the effects of Wumen Peibu Ertian ointment,which was based on the theory of tonifying deficiency and promoting circulation in meridians,on blood pressure variability(BPV)and quality of life in elderly patients with hypertension(HTN).Methods:A total of 113 elderly HTN patients were randomly divided into two groups according to the random number table method.The control group (n=56) received routine western medicine antihypertensive treatment,while the observation group (n=57) were treated with Wumen Peibu Ertian ointment in addition to routine treatment for 6 weeks.Before and after treatment,the blood pressure levels,scores of SF-36,traditional Chinese medicine (TCM) syndrome scores,and changes in TCM therapeutic effects were compared between the two groups,and adverse reactions during the treatment period were also compared.Results:After 6 weeks of treatment,in the observation group,the diastolic blood pressure standard deviation(DSD)and 24-hour average diastolic blood pressure coefficient of variation(DCV)decreased(all P＜0.05).The scores of physical functioning,role-emotional,mental health,vitality,and general health in the observation group and the score of general health in the control group increased(all P＜0.05),and the score of role-emotional in the observation group were higher than that in the control group(P＜0.05).The scores of dizziness,lumbago,knee weakness,five-heart heat,aversion to cold with cold limbs,palpitation,insomnia,forgetfulness,shortness of breath,and frequent urination at night in the observation group Dizziness,waist soreness,chest tightness,cold stomach and cold limbs,dry mouth,insomnia,amnesia,frequent night urination scores in the control group decreased(all P＜0.05).After 12 weeks of treatment,in the observation group,the scores of physical functioning,role-physical,role-emotional,bodily pain,mental health,vitality,and general health and in the control group,the scores of mental health,vitality,and general health increased(P＜0.05),and the score of role-emotional in the observation group was higher than that in the control group(P＜0.05).The scores of dizziness,lumbago,knee weakness,five-heart heat,chest tightness,aversion to cold with cold limbs,constipation,palpitation,insomnia,forgetfulness,poor appetite,shortness of breath,and frequent urination at night in the observation group and vertigo,waist soreness,chest tightness,cold limbs cold,dry mouth, cardiopalmus，insomnia,amnesia scores in the control group decreased(P＜0.05),and the score of forgetfulness in the observation group was lower than that in the control group(P＜0.05).The total effective rate of TCM therapeutic effect in the observation group was higher than that in the control group(P＜0.05).During the treatment period,no serious adverse reactions occurred in either group.Conclusion:The combination of Wumen Peibu Ertian ointment,which was based on the theory of tonifying deficiency and promoting circulation in meridians,and western medicine can effectively improve BPV and quality of life in elderly HTN patients with high safety.
