摘要:
目的:探讨针灸联合推拿对腰椎间盘突出症疗效及对患者疼痛程度的影响。方法:选取90例腰椎间盘突出症患者,按随机数字表法分为对照组、研究组,各45例; 对照组给予常规推拿治疗,研究组在对照组基础上给予针灸治疗。比较两组患者临床症状消失时间、临床疗效、疼痛视觉模拟量表(VAS)评分、日本骨科医师协会(JOA)评分、肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)、P物质(SP)、神经肽Y(NPY)水平。结果:治疗后,两组腰腿疼痛难忍、痛处固定、腰部僵硬、腰部刺痛、痛处拒按、难以俯仰症状消失时间明显缩短,且研究组短于对照组,差异有统计学意义(均P<0.05)。研究组总有效率97.78%高于对照组的82.22%,差异有统计学意义(P<0.05)。治疗后,两组VAS评分降低,JOA评分升高,研究组VAS评分低于对照组,JOA评分高于对照组,差异有统计学意义(均P<0.05)。治疗后,两组TNF-α、IL-1β、NPY、SP水平降低; 研究组TNF-α、IL-1β、NPY、SP水平低于对照组,差异有统计学意义(均P<0.05)。结论:针灸联合推拿能减轻腰椎间盘突出症患者临床症状,减轻疼痛程度,改善神经功能,临床疗效较好。
Abstract:
Objective:To investigate effect of acupuncture and moxibustion combined with massage on patients with prolapse of lumbar intervertebral disc and its influence on pain.Methods:90 patients with prolapse of lumbar intervertebral disc were divided into control group and study group,according to random number table method.The control group was given routine massage,and the study group was given acupuncture treatment on the basis of the control group.The time of symptom disappearance,clinical efficacy,visual analogue scale(VAS)score,Japanese orthopedic association(JOA),tumor necrosis factor-alpha(TNF-α)、interleukin 1β(IL-1β)、substance P(SP)、neuropeptide tyrosing(NPY)were compared between the two groups.Results:After treatment,the time of lumbago and leg pain,pain fixed,lumbar stiffness,lumbar tingling,pain refused to press,difficulty in pitch recover symptoms disappeared were significantly shortened in the two groups,and the study group was shorter than the control group(all P<0.05).The total effective rate of the study group was 97.78% higher than that of the control group 82.22%,differences statistically significant(P<0.05).After treatment,both groups' VAS scores decreased,JOA scores increased,and the study group's VAS scores were lower and JOA scores were higher than those in the control group(all P<0.05).After treatment,the levels of TNF-α, IL-1β,NPY and SP in the two groups decreased.The levels of TNF-α,IL-1β,NPY and SP in the study group were significantly lower than those in the control group(P<0.05).Conclusion:Acupuncture and moxibustion combined with massage can alleviate the clinical symptoms of patients with prolapse of lumbar intervertebral disc,reduce the degree of pain,improve nerve function,clinical efficacy is good.
